Google Forms is a useful tool to help you plan events, send a survey, give students a quiz, or collect other information in an easy, streamlined way. A Google form can be connected to a Google spreadsheet. If a spreadsheet is linked to the form, responses will automatically be sent to the spreadsheet. Otherwise, users can view them on the “Summary of Responses” page accessible from the Responses menu.

You are going to create a Google Form that could be useful in your current major or future career. Keep the theme of the form consistent with the projects you have completed thus far in the course. Below is a description of what to include and a screencast video that will show you how to create a Google Form.

Your Google Form must include:
A title and a brief description.
Five (5) questions, each being of a different question type. This means, when you use the drop down menu for types of questions to choose from, you end up choosing at least 5 different types.
You must apply a theme to your form. Don’t leave it the default purple.
You DO NOT need to collect any responses at this time.

Make sure to rename your form and that you give a meaningful name, like “A Survey on Dorm Life”. Names like “My First Form” are not acceptable.
