Purpose of Agreement: The sole intent of this Agreement is to create a positive stable
coaching relationship between the client and the coach to assess and enhance the clients
personal and professional goals. We will formulate a workable plan to execute them objectively
and successfully in respect to the time period covered within this agreement.
The Parties agree as follows:
1. Coaching Goals. Client would wish to obtain Coachs services as to achieve the
below goals and expertise to improve the clients personal life in different areas.
2. Coaching Fees. Client agrees to pay Coach the required fees for the coaching
services rendered.
3. Coaching Schedule. Coach and Client will use the following methods of contact for
meetings throughout the coaching relationship:

4. Coach-Client Relationship Duties & Responsibilities. The Client hereby engages
the Coach, and the Coach accepts such engagement to provide the following coaching
services for the Client (hereinafter, the “Services”):
Assistance with making better business decisions
Help to resolve conflicts and work effectively in groups
Increase individual and team accountability
5. Confidentiality. All information pertaining to this coaching agreement is meant to be
confidential and not to be shared to any third party. Clients information including
name and contact are not to be disclosed to anyone but should be handled and
protected with discretion.
However, the following are instances where the Coach may be obligated to break
confidentiality and release information to appropriate authorities, family members, or
a court of law:
a) If it is suspected that abuse or neglect of children or elders occurs.
b) If in my presence you make or imply threats of violence or are a danger to
yourself or another person.
c) If I feel you may lose control of your actions.
d) If something illegal is occurring.
6. Cancellation Policy. The client agrees to notify the coach of any emergencies or lastminute cancelations at least 24 hours prior to the scheduled session.
7. Termination: This agreement will come to an end after a given period of
