1. My client is a 37-year-old teacher who happens to be newly divorced. Cammy has no children
but because of her profession she typically has a very busy schedule. She typically goes grocery
shopping once a month but other than that she eats fast food. She has a family history of
obesity and heart disease. Although she doesn’t have heart disease, she has of habit of binge
eating and failed yoyo dieting. She has tried in the past to control her binge eating but doesn’t
know how. She says that her job stresses her out so once she is stressed, she eats. By the time
my client gets home from work she is usually drained which prevents her from working out. She
wants to make a change in the way she eats and how she eats however so that she can lose 30
lbs., feel and look better. She states she has por eating habits and only jogs about 4 miles on the
weekends for about 2 hours each day besides that she doesn’t do any other physical activity.
She plans to incorporate at least 10 more hours of physical activity during the week along with
dieting to lose the 30 lbs. that she wants to lose.
2. Client level and the reason for choosing this level?
Cammy Is a level 1 client because as she mentioned in her initial assessment, she stated she wanted to
lose weight so that she can look better and stop binge eating. From that I gathered she wants to start
working out for up to 14 hours a week even with her tight schedule as a teacher. She has a can do
attitude.
3. Some limiting factors that Cammy has are schedule, resources and eating to many processed
foods. Cammy is a teacher she wales up early in the morning and gets off work around 5:30 pm
by the time she gets home she is usually exhausted and too tired to prepare a healthy meal. Her
house is also in deep in the Country where there are no whole foods store so most of her
grocery shopping is done at a local Walmart which limits her to a lot of whole foods and offers
an abundance of processed foods.
4. Some behavioral goals for Cammy will include having a Can-Do attitude, weight loss goal setting
(I plan to lose 10 lbs. a month, I plan to eat less, I plan to eat a high protein low carb diet) and
monitoring progress of her weight loss journey.
5. Which assessments will record and why/how you will incorporate the results? Some
assessments, Past or current health problems (Medical History & Present Medical Condition
(Base Blood Chemistry) Body composition or weight change goal (Body Measurements) Athletic
sports nutrition (Athletic Nutrition needs Athletic Nutrition Journal) Readiness for change (
ready wiling able) Client progression (Limiting Factors, Advantages and Behavior Goals Make it a
habit)
6. Your recommended nutrition and supplement plan for Cammy.
(Research a good one and create one similar)
7. What (if anything) your client will need to discuss with his or her physician
(Cammy has a family history of heart disease and she is considered obese for her age so come
up with a discussion she should have with her doctor.)
8.
Referrals to your professional network (if needed)
9.
Proposed appointment scheduling including:
• Frequency
• What you will discuss at each appointment
10. A plan of action if you observe a plateau
(What will you do if you notice little to no change in client?)
11. A plan of action if your client changes his or her goals.
12. Be as detailed as possible in your plan.
