Explain the purpose of your interview, gain consent, and ask the individual pertinent questions about their disability, the challenges they face as a result of living with the disability, coping mechanisms or what strategies or tools they find helpful in helping them to live with this condition, what their emotional, physical, and/or spiritual needs may be, and any other questions you might want to ask.
Be sure to include demographics such as age, name of the disability, length of time the person has lived with the disability, whether or not they have a caregiver, etc. You may document some of the time used for the interview on your practicum log.
